Why doesn't everyone recieve the banned text? Only the error code..

I am trying to figure this on out.

Where does this message come from, Live servers or is it stored in the console? 

If it is from the Servers then why do some people not see this message? 

I have a Hitachi drive and all I get is a blank screen.  I have to press the "y" key to see the error code.

Could this possibly mean that the console is blocking me from getting on the Liver servers instead of the Live servers blocking and sending the response?
